In the long history of Bug 1027903, creating the component for the form to file bugs into was overlooked. We need to create that component now.

Per the normal policy, I'm making the request to create a form with the following parameters:


    The name of the component: WebOps: Request
    The product it should go in: Infrastructure & Operations
    A longer description: Requests for the WebOps team.

I'm also needinfoing the appropriate person to make sure this is correct.
Or we can remove the form, either way this bug must exist.
