When configuring an additional SMTP server, selecting the "advanced" button
brings up a form to enter the additional server. The form's "cancel" and "OK"
buttons don't work. The form can't be exited, Mozilla is frozen and must be killed.

Reproducible: Always
Steps to Reproduce:
1.select "view settings for account"
2.select "outgoing SMTP server"
3.select "advanced"



Actual Results:  Mozilla freezes

Expected Results:  allowed an additional SMTP server to be added
