The heartbeat survey model doesn't have a description which makes it hard for Gregg to know which survey is which.

This bug covers adding a description field. We can default it to empty string or null--doesn't matter. Then we need to add that to the SurveyAdmin and the survey view in the analyzer section.

Talked to lguruprasad in IRC and he threw together a PR already.
